Enter in your CollaborateMD Patient Account Number.
Unsure what your CollaborateMD Patient Account number is? Your Account Number can be found in the top right corner of your statement. If you can't sign in, please contact your physician's office.
- Click Next.
Select Create an account to see your balance and history.
Enter your Last Name, Date of Birth, and the last 4 of your SSN or Member ID Number.
Click Next.
Enter your email, then click Next.
Copy the Verification Code from your email, then click Next.
Create a secure Password.
Click Register.
