Practice Data
Practice Data holds all patient information under a customer's account. All of the fields under this category correspond directly with fields found under the Patient section.
| Name | Description |
| Practice ID | The unique identifier for a Practice, as seen in the "Seq#" field. |
| Code | Associated with the "Code" field. |
| Associated with the "Email" field. | |
| Flu Shots ID | Associated with the "FL Shots ID" field. |
| Inactive? | Displays "Yes" if the "Click Here To Make Practice Inactive" option i s selected. Otherwise, this field will display "No". |
| NPI | Associated with the "NPI" field. |
| Name | Associated with the "Name" field. |
| Name w/ID | Displays the Practice Name with it's ID. Example: Main Practice (12345678) |
| Organization Type | Displays the selected option in the "Organization Type" selection field. |
| Reference Number | Associated with the "Reference #" field. |
| Taxonomy Code | Displays the taxonomy code of the selected option in the "Taxonomy Specialty" selection field. |
| Fax Number | Associated with the "Fax#" field. |
Main Address
The Main Address sub-category, found under Patient Data, corresponds to the main address information set on practice records.

| Address Line 1 | Associated with the first "Main Address" field. |
| Address Line 2 | Associated with the second "Main Address" field. |
| City | Associated with the "City" field. |
| State | Associated with the "State" field. |
| Zip Code | Associated with the "Zipcode" field. |
Pay-To Address
The Pay To Address sub-category, found under Practice Data, corresponds to the pay-to address information set on practice records. All of the fields under this category correspond directly with fields found within the Practice section of the application..

| Address Line 1 | Associated with the first "Pay To Address" field. |
| Address Line 2 | Associated with the second "Pay To Address" field. |
| City | Associated with the "City" field. |
| Is Pay-To Same? | Displays "Yes" if the "Pay-To Address is the same as the primary office address" option is selected. Otherwise, it will display "No". |
| State | Associated with the "State" field. |
| Zip Code | Associated with the "Zipcode" field. |
