Answering Security Questions

If you haven’t configured your user profile, you won’t be able to reset your password via email or security questions.  Please contact your Admin or Auth Rep.

  1. Visit app.collaboratemd.com

  2. Click on the Forgot your Password? link.

  3. On the next page, enter your CollaborateMD Username.

    Your username is usually your first and last name with no spaces (ex. janedoe).

  4. Click Lookup User.

  5. Enter your answer for each question in the fields provided and click the Log In button.

    The answers to the security questions must match exactly (i.e., punctuation, spacing and capitalization).

  6. You'll be prompted to change your password.
