If you've forgotten the password you use to sign in to CollaborateMD, you can reset it easily at any time.
For security measures, you can't reuse the same password. Your new password must be a minimum of 8 characters long, with at least 1 upper case letter, 1 lower case letter, 1 number, and 1 special character.
Forgot your password? Depending on whether or not you're signed in to CollaborateMD, there are two ways to reset your password.
If you haven’t configured your user profile, you won’t be able to reset your password via email or security questions. Please contact your Admin or Auth Rep.
Visit app.collaboratemd.com.
Click the Forgot your Password? link.
On the next page, enter your CollaborateMD Username.
Your username is usually your first and last name with no spaces (ex. janedoe).
Click Lookup User.
Select Reset my password via email.
Check your email inbox for a one-time verification code.
Return to the reset password page and click I have a verification code, populate the code and click the Log In button.
You'll be prompted to change your password.
