Important: Legal documentation must be provided in order for CollaborateMD to process your request. If the required fields and/or confirmation checkboxes are not completely filled out, CollaborateMD will be unable to process the request.
Select Account Administration > Account Management.
Click the Edit Account Information link.
Click the Request Account Name Change link.
Once the Request Account Name Change window is displayed, enter the following required information:
New Account Name
New Account Phone Number
New Account Email Address
Click the Next button.
Check the box confirming you understand and agree to send the required Legal Documentation.
Click the Next button.
Check the box confirming you have fully read and agree the Customer Software Agreement and the Business Associate Agreement.
Click the Submit button.
Send your document(s) to the Accounting Department via fax at (888) 881-1812 or email them to accounting@collaboratemd.com.

Follow the steps below to request to change the owner of the account.
Select Account Administration > Account Management.
Click the Edit Account Information link.
Click the Request Ownership Change link.
Once the Request Ownership Change window is displayed, enter following required information:
New Owner First Name
New Owner Last Name
New Owner Phone Number
New Owner Email Address
Click the Next button.
Check the box confirming you understand and agree to send the required Legal Documentation.
Click the Next button.
Check the box confirming you have fully read and acknowledged the Customer Software Agreement and Business Associate Agreement.
Click the Submit button.
Send your document(s) to the Accounting Department via fax at (888) 881-1812 or email them to accounting@collaboratemd.com.
In the event you change your business name, or there is a change in ownership, you may be required to update your account Tax ID.

Select Account Administration > Account Management.
Click the Edit Account Information link.
Click the Request Account Tax ID Change link.
Once the Request Account Tax ID Change window is displayed, enter the New Tax ID in the field.
Click the Next button.
Check the box confirming you understand and agree to send the required Legal Documentation.
Click the Next button.
Check the box confirming you have fully read and acknowledge the Customer Software Agreement and Business Associate Agreement.
Click the Submit button.
Send your document(s) to our Accounting Department via fax at (888) 881-1812 or email them to accounting@collaboratemd.com.
If your office has moved to a new location, make sure the address is updated within CollaboarteMD. Follow the steps below to edit the account contact address.
Select Account Administration > Account Management.
Click the Edit Account Information link.
Click the Edit Address link.
Make the necessary updates.
Click the Save button. Otherwise, click Close if no changes were made.
